目的 分析丁苯酞对老年急性脑梗死(ACI)患者肢体和神经功能恢复预后的影响。方法 选取130例老年ACI患者，随机分为观察组和对照组各65例。观察组在对照组治疗基础上给予丁苯酞注射液。比较两组神经、认知、肢体功能和血清神经功能指标水平。结果 治疗后，观察组美国国立卫生研究院脑卒中量表(NIHSS)评分显著低于对照组(P<0.05),Barthel指数评分、蒙特利尔认知评估(MoCA)量表评分和简式Fugl-Meyer运动功能测评法(FMA)评分显著高于对照组(P<0.05)。治疗后，观察组血清神经元牧场划性烯醇化酶(NSE)、神经肽苯(NP)Y、S100β、髓鞘碱性蛋白(MBP)水平显著低于对照组，对照组恶化患者比例显著高于观察组(P<0.05)。观察组总有效率显著高于对照组(P<0.05)。MBP与NIHSS评分呈正相关，与Barthel评分、MoCA评分、FMA评分呈负相关(P<0.05)。结论 丁苯酞可以降低神经细胞因子水平，促进神经功能、认知功能和肢体功能恢复。
