目的 探讨中国老年人群中内脏肥胖指标对慢性肾脏病(CKD)的预测价值。方法 对上海市闵行区接受国家基本公共卫生服务(老年人健康管理服务项目)中62 774名老年人采用Spearman相关分析和多因素Logistic回归分析内脏肥胖指标与CKD之间的关系,采用受试者工作特征(ROC)曲线评价不同内脏肥胖指标对CKD的预测价值。结果 男性和女性CKD的患病率分别是18.41%和21.89%。内脏肥胖指标与CKD患病存在关联,并且在调整了混杂因素后这种关联仍然存在,内脏肥胖指标评分增加,老年人患CKD的风险增加。ROC曲线分析显示,男性和女性中内脏肥胖指标[内脏脂肪指数(VAI)、新VAI(NVAI)、中国VAI(CVAI)、腰围(WC)、脂质蓄积指数(LAP)、体形指数(ABSI)、体圆度指数(BRI)、胰岛素抵抗代谢评分(METS-VF)]与CKD的AUC均>0.5(P<0.001),其中NVAI与CKD的AUC最大,男性AUC为0.620(95%CI:0.617~0.628)和女性AUC为0.611(95%CI:0.604~0.618)。结论 内脏肥胖指标对CKD具有一定预测价值,其中NVAI的预测价值最高。
