目的 探讨不同体质量指数(BMI)的2型糖尿病(T2DM)患者血清鸢尾素(Irisin)变化及与炎症反应的关系。方法 选取T2DM患者241例,根据《中国成人超重和肥胖症预防控制指南(试行)》分为糖尿病正常体质量(A)组(62例)、糖尿病超重(B)组(84例)、糖尿病肥胖(C)组(95例)。对3组临床资料、Irisin水平及炎症因子水平进行比较。多因素回归分析Irisin与肥胖的关系及Irisin对BMI和炎症因子的中介效应,采用LOWESS法分析Irisin与炎症因子的曲线关系。结果 C组BMI、空腹胰岛素(FIns)、胰岛素抵抗指数(HOMA-IR)、三酰甘油(TG)、甘丙肽(Galanin)水平明显高于A组和B组,A组和B组Irisin水平明显低于C组(P<0.05)。炎症因子方面,C组C反应蛋白(CRP)、单核细胞趋化蛋白(MCP)-1、纤容酶原激活物抑制物(PAI)-1、白细胞介素(IL)-6水平明显高于A组和B组,转化生长因子(TGF)-β1水平明显低于A组和B组(P<0.05)。多因素回归分析结果显示,CPR、MCP-1、TGF-β1、PAI-1、IL-6与Irisin水平增加显著相关(P<0.001)。Irisin水平与肥胖风险呈显著正相关,随Irisin水平上升,患者肥胖风险显著上升。结论 Irisin水平越高,患者炎症反应越重,肥胖风险也会越高。
