<正>老年患者常合并多种基础疾病,心肺等重要器官功能储备显著下降,使得围术期管理面临巨大挑战。加之全身麻醉本身即可通过多种机制诱发肺不张,包括功能残气量(FRC)下降、膈肌功能抑制、呼吸道分泌物滞留及吸收性肺不张等。据报道,高达90%的患者在麻醉诱导后数分钟内即可出现影像学可见的肺不张,而老年患者由于胸廓顺应性降低、肺弹性回缩力减弱及闭合气量(CV)增加等生理性老化改变,更易形成顽固且广泛的肺不张[1]。
