目的 探讨单核细胞/高密度脂蛋白胆固醇比(MHR)联合超敏C反应蛋白(hs CRP)对老年冠状动脉慢血流(CSF)患者早期诊断的预测作用。方法 入选2023年6月至2024年4月因胸痛或胸闷症状就诊河北北方学院附属第一医院心内科,年龄≥60岁,行冠脉造影证实CSF组40例,冠脉血流正常组(NCF) 40例。收集病史及化验指标等基本临床资料,比较两组差异。分析CSF组MHR与hs CRP、MHR与矫正TIMI计帧数(c TFC)之间相关性,使用Logistic回归分析MHR和hs CRP是否是CSF的独立危险因素,利用受试者工作特征(ROC)曲线评估二者联合对CSF的预测价值。结果 两组高脂血症史、吸烟史存在统计学差异(P<0.05),CSF组MHR、hs CRP、白细胞计数(WBC)、三酰甘油、c TFC显著高于NCF组(P<0.05),且MHR与hs CRP、c TFC均独立正相关(P<0.05)。Logistic多因素分析提示MHR和hs CRP均为CSF的独立危险因素(P<0.01),MHR联合hs CRP诊断CSF的ROC曲线下面积(AUC)为0.951(P<0.01),灵敏度为0.900,特异度为0.925,95%CI(0.904~0.998)。结论 MHR联合hs CRP在冠脉造影前对老年冠脉慢血流有较好的早期预测价值。
